Registration Waiver

  WAIVER: Mat-Town Wrestling Club Camps at Liberty Arena COVID-19 RELEASE FORM COVID-19 RELEASE: Assumption of Risk, Release and Waiver of Claims,
I am a wrestler and participant at the Mat-Town Wrestling Camp. I understand that the nation is in the midst of the COVID-19 pandemic, which presents certain unusual health risks that are highly publicized, including fatal illness. I voluntarily chose to participate in a wrestling academy/clinic/camp sponsored and organized by the Mat-Town Wrestling Club. In consideration for my participation therein, I, on behalf of myself, my personal representatives, heirs, and assigns, hereby:
1. Acknowledge, understand, and agree that there are certain inherent risks and dangers associated with my participation in wrestling at a Mat-Town WC Camp/Practice, including but not limited to the risk of contracting COVID-19, I knowingly and voluntarily accept and assume full responsibility for each of these risks and dangers, as well as all other risks and dangers that could arise out of or occur during my participation in or association with the Mat-Town Wrestling Club or Liberty Arena.
2. Acknowledge, understand, and agree that at all times that I am participating at the Mat-Town WC/Liberty Arena, I will use my best efforts to comply with any and all instructions provided with respect to maintaining my health and safety, including following CDC Guidelines (e.g., washing hands often, use of hand sanitizer, proper social distancing where possible, wearing appropriate personal protective equipment, covering mouth when coughing or sneezing, and staying home if not feeling well). 3. Acknowledge that, in the event a wrestler, including the undersigned wrestler, must leave due to illness, including COVID-19, that the remaining wrestlers, including the undersigned wrestler may continue to participate at The Mat-Town Wrestling Camp at their own risk. Further acknowledge that The Mat-Town WC cannot, and will not, cease its business activities including Mat-Town WC Camps, each time a wrestler appears to be ill.
4. Acknowledge that The Liberty Arena/Mat-Town WC is not a healthcare provider and diagnose or treat illnesses. Consequently, The Mat-Town WC will not diagnose or treat illnesses at their Camps/Clinics/Academy.
5. WAIVE, AND FOREVER DISCHARGE The Mat-Town Wrestling Club/The Liberty Arena(and its respective owners, members, agents, employees, counselors, clinicians, servants, officers, directors, successors, assigns and affiliates) (“Releasees”) from any and all claims, demands, actions, or causes of action I, or anyone on my behalf, may hereafter have against Releasees for loss or damage of any kind (including personal injury, illness, disability, and/or death) which occurs because I contracted COVID-19 while participating with or around Mat-Town WC, or any of the events and activities associated therewith, whether caused by Releasees’ own negligence or otherwise.
I expressly agree that this Assumption of Risk, Release and Waiver of Claims is intended to be as broad and inclusive as is permitted by the law of the State of PA or any other state’s laws under which this Agreement may be construed and that if any portion of this Agreement is held to be invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

Mat-Town Wrestling Camps/Clinics Parental Authorization and Release Liability Release: I, the undersigned, individually and as a parent/guardian of the child named on this form, a minor, ask that he/she be admitted to participate in the sports camp sponsored by the Mat-Town Wrestling. I do hereby agree to release, discharge and hold harmless Mat-Town Wrestling, The Liberty Arena, their owners, agents and employees of and from all causes, liabilities, damages, claims or demands whatsoever on account of any injury or accident involving the said minor arising out of the minor’s attendance at the sport camp or in the course of competition and-or activities held in connection with the sport camp. The supplied health history is correct as far as I know, and the person herein described has permission to engage in all prescribed camp activities. I give full permission to the camp to medically treat my child. In the event I cannot be reached in an EMERGENCY, I hereby give permission to the camp medical personnel to administer medication. I also give permission to the physician selected by the camp director to hospitalize, secure proper treatment for, and to order injection, anesthesia or surgery for my child as named above.
Overnight Permission The undersigned parent/guardian agrees to give my permission for the child named on this form to stay overnight at Mat-Town Wrestling in the student residence hall under the supervision of Mat-Town Wrestling camp staff (if designated). If hotels are used, parents and school coaches will be in charge of all supervision in between sessions and at overnight accommodations.
Photo Release Form The undersigned parent/guardian agrees to give permission to Mat-Town Wrestling to use photographs of the child named on this form for the purpose of publicizing the Mat-Town Wrestling in either general Mat-Town Wrestling promotions, which could include the Mat-Town Wrestling Web site; publications which include the print admissions package, brochures, magazines, video, television, newspaper, newsletters, and/or publications that may act as fundraising ventures for Mat-Town Wrestling clubs/organizations. The photo will most likely not contain a caption identifying any individuals, although one may occasionally accompany the picture.
Participant Code of Conduct It is expected that all participants in any University or non-University sponsored program, activity, or service will conduct themselves in a polite, respectful manner and will adhere to all University rules as follows.
A. The possession or use of alcohol and other drugs, fireworks, guns and weapons is prohibited.
B. The use of skateboards is prohibited.
C. No violence, including sexual abuse or harassment, will be tolerated.
D. Hazing, bullying, and cyber bullying will not be tolerated.
E. All curfews, if applicable, will be followed.
F. Misuse or damage of University property is prohibited.
G. All minors will be accompanied by another minor at all times. This is the buddy system.
H. Participants in an overnight program are not permitted to be housed in the same room with an
adult unless the person is the minor’s parent or legal guardian.
I. Minor participants may only be housed with a participant of the same sex.
J. No minor will be housed in a room alone.
K. Smoking is prohibited in all University buildings.
L. The inappropriate use of cameras, imaging, and digital devices is prohibited including use of such devices in showers, restrooms, or other areas where privacy is expected by participants. M. Profanity is prohibited.
N. When crossing streets, only cross in the designated crosswalks.
O. Only use the building designated by your program supervisor or staff.
P. If you are hurt or injured, immediately report your injury to the program supervisor or staff.

I understand that there are no refunds that are not approved and requested by the tournament director and by accepting this waiver I understand that my knowledge of this policy will be used on behalf of the tournament if I request a chargeback from my credit card company.ā€¯
